- Must reside within the Thornapple Kellogg School District.
- For graduating seniors or approved Michigan home school seniors.
- Minimum of a 2.5(GPA) required.
- Must be pursuing a degree in the medical/allied health field; if no qualified medical/allied health major applies a 4-H participant will be considered.
- Financial Need Required.
- Students must submit one letter of recommendation from a teacher and one from a 4H leader stating why the student is suitable to receive scholarship award.
- The preferred student will be chosen based on the following considerations: school and community activities, work experience, essay content, 4-H involvement.
